York-based health group gets $1M grant

AF4Q-SCPA, one of 16 branches around the country, is an initiative of Healthy York County Coalition created in 2007 that serves York and Adams counties. This funding marks the fourth and final phase of AF4Q, which is intended to improve health care by engaging patients in their care; publicly reporting on the quality of care provided by local physicians and hospitals; implementing quality improvement programs in doctors’ offices and hospitals; and exploring new payment methods to promote high-quality care.
AF4Q-SCPA’s Patient Partner program has 56 patients who provide feedback on provider communication, how patients think and ways to teach patients to self-manage their care. Additionally, AF4Q-SCPA’s I Can! Challenge program, a 12-week series focusing on improving the health of residents with chronic diseases through healthy eating, exercise and stress management, has graduated more than 100 residents since 2009.
With the new funding, AF4Q-SCPA will launch a new six-session series, “It’s Your Health… Take Charge!” to help address unanswered patient questions regarding health care and where to go when they need care. AF4Q-SCPA also will continue to expand its Patient-Centered Medical Home Collaborative, which brings together practices and coaches to reduce hospital readmissions, emergency department visits and preventable hospitalizations in 37 practices.
Additionally, AF4Q-SCPA will work with regional federally qualified health centers to focus on diabetes care among complex patients.
